Referral pathway

  1. Source of referral: You can self-refer to the Doncaster IAPT service by telephone or online referral form. You can also be referred by your GP, or by another healthcare professional.
  2. Primary care-IAPT: Receive your referral and arrange an appointment with a member of the team in Doncaster.
  3. Appointment-initial assessment: You will conduct an initial assessment with a psychological wellbeing practitioner (PWP). If, after the initial assessment we are not the right service for you, we will aim to find and signpost to the most appropriate service that can meet your needs.
  4. Treatment begins: The most appropriate treatment is offered and commenced dependent on your needs. Treatment offered can range from low-intensity cognitive behaviour interventions; guided self-help, or high-intensity interventions such as cognitive behavioural therapy (CBT) or counselling.

The IAPT service is not suitable for people assessed with significant risks or those whose primary issues are:

  • schizophrenia
  • bipolar disorder
  • personality disorder
  • anger management
  • obvious signs of psychosis
  • organic disorders of the brain
  • substance misuse (unless stable or on a treatment regime).

We observe strict NHS standards of confidentiality and you have control over who else is involved in your care. The only time we will inform others without your permission is if we are concerned for your immediate safety, the safety of others or any safeguarding concerns.

During telephone assessment a practitioner will ask whether or not you consent for us to share information with other services.

You can give your consent at different levels, and it’s important to make your choice clear to a therapist:

  • Full consent: GP and NHS will have access to your medical records. This is the typical level of consent within the NHS, and is so because of the benefits to your healthcare provision. The benefits include increased insight into what has worked best for you in the past, and allows for increased effectiveness of multidisciplinary care.
  • Restricted consent: Your clinical record will only be accessible by the IAPT service.
